We have recently revised our health and trip information process to better serve the parent's and the school's needs. The school will contact you three months before your health forms are due which will coincide with your child's annual physical. Follow the directions on the both the letter and the form. We have streamlined the process so you do not have to spend a great deal of time filling out information.
